Who is Jacob supposed to be lost?
Apparently Jacob is a Judeo-Christian God-like figure, the “others” and Richard are his angels, and the survivors of Oceanic Flight 815, like many a vessel previously crashed at the island by Jacob, are his human flock.
There has always been an evil on the island - The smoke monster is the evil. It seems that the island has always housed both the light and the evil... The evil judges the people who come to the island - per whoever is in charge (most of which occurred during Jacob's time as protector).

In Across the Sea, it's finally established that the Smoke Monster was created when Jacob pushed his mortal brother into the Heart of the Island. The Man in Black's physical body went in, and the Smoke Monster emerged in a clangorous roar.
